This session will focus on the collaborative and interdisciplinary nature of improving the quality of care and life for PA/LTC residents by looking at a nationally recognized facility as a model of excellence in providing high quality care, and a QI project on antibiotic stewardship by SeaView Hospital Rehabilitation Center, the 2018 QIHO award winner.

Edgemoor DP a 192-bed county-run safety net facility cares for individuals with complex medical, behavioral and psycho-social needs received the 2017 AHCA/NCAL Gold Level Quality Service Award. Their medical director Rebecca Ferrini, MD, MPH, CMD will discuss this award and the Baldrige criteria, describe how the facility achieved excellent outcomes for its residents and staff, and provide tools you can implement to develop systems to create a top tier organization.

Learning Objectives:
(1) Appreciate the diversity and impact of quality improvement and quality of life projects in the PA/LTC setting.
(2) Describe the difference in project design and implementation of quality of care and quality of life projects.
(3) Summarize the Baldridge categories and criteria and how they apply to the nursing home environment.
(4) Analyze your organization’s systems or overarching plans for relationship management, communication and safety separately from the policies and procedures you have developed in those areas.
